Dear Patient

I am pleased to inform you that Sanjay Aggarwal MPA will be acquired by MyCHN effective January 2, 2025, to bring you more services and improved hours of care. This new relationship brings access to affordable, name brand prescription medications through Community Health Networks’ pharmacy. Dr. Aggarwal will continue to provide high quality care through his work with MyCHN. Dr. Sanjay Aggarwal will retain hospital privileges to all the hospitals he is currently affiliated with. We hope you will continue your care with Dr.Sanjay Aggarwal and MyCHN.

MyCHN is a federally qualified health center that has been in existence since 2008. MyCHN provides high-quality, whole-person medical, dental, and behavioral health services, including psychiatry, for patients. As a patient, you and your family members are welcome to use these services. MyCHN accepts Medicare, most Private Insurance, and Medicaid and sees patients with no insurance. Any patient who does not have insurance may qualify for services for a small copay based on household size and income.

Once the merger is effective on January 2, 2025, the name of SanjayAggarwal MDPA practice name will change to CHN Specialties. Please note that the staff and practice will only access your medical records if you consent to become a patient of the practice. You will need to consent for your medical records to be released to MyCHN Specialties. If you are not due for an appointment, please complete the attached medical record release so your medical information may be added to the MyCHN Specialties electronic health record in preparation for your next visit.
